Care Manager provides ongoing, hands-on support to ensure a participant’s care plan is followed and adapted as needed. They coordinate services, advocate for the participant, and monitor day-to-day progress.
Support Coordinator, on the other hand, mainly helps connect participants with providers and services under their NDIS plan, but may not provide regular follow-up or personal case management.

Yes, we work with participants with low, medium, and high support needs, including those in SDA (Specialist Disability Accommodation) and high-intensity daily personal activities. We tailor our care management approach to meet each person’s unique goals and requirements.

Absolutely! We believe in a collaborative approach and are happy to work closely with your existing Support Coordinator, allied health providers, or case managers to ensure a seamless support experience.

Yes. Care Management tasks can often fall under Core Supports or Capacity Building (such as Improved Daily Living or Coordination of Supports). We can help you review your plan and determine how our services can fit.
